Empowering students to reach a higher state of health and well-being.

Healthy Transitions provides curriculum for young adults transitioning out of foster care to empower them to recognize how stress, anxiety and past traumas impact their daily lives, and how identifying and managing these factors can increase their success in life. 

The Healthy Transitions curriculum and training activities focus on increasing awareness and disseminating information about the many facets of wellness and self-care, with the goal of instilling these students with healthy habits, connecting them to local resources and services, and empowering them to reach a higher state of health and well-being. Through a combination of training and peer-to-peer support, Healthy Transitions serves to support these young people physically, emotionally, socially, and spiritually. The Healthy Transitions Curriculum includes youth workbooks, supplemental workbooks, and student resource guide. The trainer program components include a training guide, presenter slides, supplemental slides, peer to peer support group manual, and coordinator resource tools.

The Healthy Transitions is funded by the voter-approved Mental Health Services Act (Prop. 63) and is a partnership between the California Community Colleges Chancellor’s Office and the Foundation for California Community Colleges.

Program Funding

The Foundation thanks the California Mental Health Services Authority (CalMHSA) for their financial support of this project. CalMHSA is an organization of county governments working to improve mental health outcomes for individuals, families, and communities.

 

Prevention and Early Intervention programs implemented by CalMHSA are funded through the voter-approved Mental Health Services Act (Proposition 63). Proposition 63 provides the funding and framework needed to expand mental health services to previously underserved populations and all of California’s diverse communities.

What Students are Saying

This class has helped me by showing me that there’s also other kids in my struggle. They also talk about helpful subjects that become useful in the future after foster kids get out. I learned also how to control my anger and stress better. Being around foster kids and other kids in my situation makes me feel not so alone. I like these classes.

I feel great about going to college. The best part about the [Healthy Transitions] program is it helps with meeting people that share similar stories as me. The program also provides great resources to equip you for a better future. The Program taught me that helping others is good for releasing stress and exercise as well. I’m studying Mechanic Engineer and plan to transfer to USC. YESS has helped me to stay consistent with attending college.
